Return to School Covid Forms

To help prevent the spread of COVID-19 in schools, every parent/guardian, or student over 18 years must complete and submit this form when returning to school after each period of closure such as school holidays or after any absence from school. On review of the form, school management may contact you and ask that the student does not return to school immediately and will discuss a suitable future date for return. N.B. Every question must be answered.

Please note: This sensitive personal data is being collected for the purposes of maintaining safety within schools in light of the Covid-19 pandemic. The legal basis for collecting this data is based on vital public health interests and maintaining occupational health and will be held securely in line with our retention policy. Form content is extracted from the HSE and HPSC (Health Protection Surveillance Centre) return to work safely guidance and DES Covid-19 Response Plan for the safe reopening of schools.
